To develop, coordinate, implement and direct person directed lifestyle programming for residents with cognitive impairment; to provide a therapeutic environment that strives to enhance each resident’s physical, social and psychological well-being by creating moments of success.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  1. Adhere to all Sonata objectives, policies and procedures
  2. Attend department head meetings, Lifestyle departmental meetings and Memory Care care meetings
  3. Prepares and provides the Executive Director with records and reports regarding progress toward stated goals
  4. Provides Executive Director reports and records as required for expense and budgeting purposes
  5. Responsible for the creation of monthly programming calendars
  6. Responsible for the development, planning and implementation of the Memory Care Lifestyle activity program on time and as scheduled
  7. Responsible for interview, training, scheduling and overseeing Memory Care Activity Assistants
  8. Responsible for initial orientation and ongoing education of Person Directed Care, PAC Training, initiating activities, topics of discussion, communication techniques, etc.
  9. Participates in the interviewing and supervising of Serenades Resident Assistants.
  10. Review Lifestyle profiles and maintain astute knowledge of residents’ profiles and preferences, ensuring Memory Care team is knowledgeable of content
  11. Record daily residents’ attendance at activities
  12. Monitor grooming of residents and assists if necessary
  13. Treat residents with dignity and observes all residents’ rights and confidentiality
  14. Communicate with team regarding changes in resident’s condition and/or preferences
  15. Hosts and oversees smooth operation of individual and group activities and events as scheduled
  16. Shops for supplies as needed
  17. Follows all vehicle safety policies and procedures
  18. Drives company car for doctor appointments, or other memory care resident transportation needs if Driver, or other Lifestyle team members are not available
  19. Maintains friendly and concerned relationship with residents while upholding the Company’s policies and procedures
  20. Coordinates with families and staff on the development of the Person Directed Care Plan as it relates to the resident’s preferred lifestyle, activity interests and personal preferences
  21. Is considered and utilized as the dementia care and Alzheimer’s Disease community expert
  22. Utilizes expertise to develop and enhance professional referral sources and provide education to the community as needed.
  23. Maintains astute knowledge of Memory Care program components and product differentiation
  24. Acts as a liaison facilitating communication between Resident Care, Memory Care team members, families, community management, and others
  25. Provides for therapeutic environment in the residents’ personal and congregate areas, compensating for cognitive, sensory and physical impairments
  26. Provides individual or group services for the residents and/or families at times of adjustment or crises and as programmed on a regularly scheduled basis
  27. Provides, as needed, resident and family services such as counseling, problem-solving, referral and advocacy
  28. On call for emergencies such as hurricane or other major emergencies
  29. Participates in tours, presentations or other sales events as appropriate

  • Maintains a safe and secure working environment and practices safe working habits.

  • Complies with the cell phone and social media policies.
  • Maintains neat appearance, good personal hygiene and appropriate attire per company dress code.
  • Participates in training, in services and attends meetings when required.
  • Assists with special projects as assigned and performs other related duties.
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Prior to presence in the community, applicant must have completed a Post Hire Health Questionnaire upon offer of employment. A TB test result must be received with 7 days of employment. Previous TB test results cannot be older than 6 months; chest x-ray results 2 years. A Criminal Record Clearance is required prior to team member’s starting date.

EDUCATION and/or EXPERIENCE
Must be 18 years of age or older and have graduated from high school. College is preferred. Advanced training and experience in Alzheimer’s or related dementia are desirable and one year or more work experience in home care for the elderly or Long Term Care is preferred. Knowledge of or ability to learn about various disabilities such as memory impairment, Parkinson’s disease, arthritis as well as others as population dictates is preferred.

LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence is required. Ability to speak effectively before groups of customers or team members of organization is required.

MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs is required.

REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations is required.

CERTIFICATES, LICENSES, REGISTRATIONS
Must have Alzheimer’s I and II certificate and / or proof of training (State Specific).

Must carry and maintain current CPR and first aide certificate.

OTHER SKILLS AND ABILITIES
Proficient with MSWord, MSExcel, MSPowerPoint, MSOutlook, and other systems knowledge is helpful. Basic typing skills essential.

PHYSICAL DEMANDS

Position requires the ability to stand, reach, bend/stoop/crouch, push/pull/lift/carry up to 50lbs, twisting/ turning and repetitive motion

Must have good vision, hearing and speech
